Neuroprotective Potential
Paradoxically, while high-dose THC carries cognitive risks in young users, some preclinical and early clinical research suggests low-dose THC may have neuroprotective effects in aging brains. A landmark 2017 study published in Nature Medicine by Bilkei-Gorzo et al. found that chronic low-dose THC treatment reversed age-related cognitive decline in mice, restoring the cognitive performance of 12-month-old mice to that of 2-month-old controls [1]. The mechanism involved CB1 receptor-mediated restoration of hippocampal synaptic plasticity.
Whether this translates to humans remains a critical unanswered question. A small Israeli pilot trial of low-dose THC in adults over 65 with treatment-resistant behavioral symptoms of dementia found improvements in function with minimal adverse effects, but the sample was too small (n=10) to draw conclusions [2].
Cognitive Risk in Older Adults
The same cognitive risks documented in adolescent and young adult users — working memory impairment, processing speed reductions — may manifest differently in older adults. Drug interactions are a significant concern: cannabis interacts with warfarin (increases bleeding risk), statins, and benzodiazepines. Falls risk may increase with THC intoxication, which has outsized consequences in the elderly.
